•  Nightingale Care LLC, USA will be the first point of contact for sponsors who wish to enroll for this annual subscription program.
• All preliminary queries will be attended to by e-mail.
• Sponsors will be required to complete enrollment forms and deposit the annual subscription fees.
• Now partner hospital shall start communicating with sponsor and beneficiary to commence the services that comes with the subscription.
• Nightingale Care shall help in all possible ways to bridge any communication gap with partner hospital but will not be part of any matters related to medical treatment.
• Sponsors are welcome to contact Nightingale Care to express their concerns and utmost effort will be made to sort out any grievances.
• Nightingale Care will be glad to provide you suggestions regarding health care services in Kolkata, especially with partner hospital

This Subscription Program brings to you the following services through the Partner Hospital

• Dedicated Care Manager and Primary Care Physician
• One Complementary Annual Health Check-up
• Annual dietician & optometrist consultation
• Annual Check-up by in-house Cardiologist.
• Medical History, Documentation, Patient File Creation with Transparent Billing System.
• 2 complementary video or in-person consultations with Primary Care Physician
• Complementary delivery of medicines within 5Km radius of the hospital, beyond 5Km medicine may be delivered with applicable charges
• Discount on Bed and Investigation charges
• Home Collection of samples
• Report delivery by e-mail/WhatsApp
• During hospital visit Care Manager shall escort and extend all services from parking to billing to follow-up visits.
• Dedicated waiting area to avoid hospital rush. All formalities will be taken care of by Care Manager. No waiting in reception/billing queues.
• Priority booking with minimum waiting time
• Care Manager to attend emergency on phone and arrange hospital visit with guaranteed bed availability

In case of Emergency

• Care Manager shall arrange transfer to hospital
• Inform sponsor by phone or WhatsApp
• Make all necessary arrangements to ensure required treatment is provided
• Sponsor/beneficiary must transfer payment, within 24 hours of information receipt, based on estimated amount shared by Partner Hospital
• Care Manager shall arrange all implants, lenses, blood etc, as prescribed by physician.
• Sponsor will be kept informed for all such medical activities by suitable means
• Sponsor must encourage Care Manager to do the needful for the best possible medical services extended to the beneficiary
• Before discharge of patient, sponsor or other designated person must clear all bills
• If requested Care Manager shall make logistic arrangement at additional charge.
• In-house 24 hrs pathology and radiology services.
• We provide 24 Hours of Ambulance Service with trained emergency medical team.

Other than the sponsorship fee, no financial transaction will be carried out with Nightingale Care. Nightingale Care LLC is a means of providing comfort and care for your loved ones. An assurance that they will be looked after in times of need.
